New Passport Application Requirements

You are eligible to apply for a new U.S. passport if:

  • You have never been issued a U.S. passport
  • Your expired U.S. passport was issued more than fifteen years ago
  • Your expired U.S. passport was issued when you were under age 16
  • You cannot legally document your name change from your previous U.S. passport

Step 2:

New Passport Application Checklist

For your convenience we provide you with a New Passport Checklist to keep track of all the documents you will need to apply for a new passport.

  1. DS-11 Form. You must complete the DS-11 online. Print two copies. Do not sign now.
  2. Travel Itinerary. If you have a travel itinerary for your upcoming trip, please include it in addition to your other documentation. Travel plans include: a business letter, hotel reservations, flight schedule, etc. Please print two copies.
  3. Passport Photo. Your passport must meet all of the government requirements. Please do not wear headwear, tinted glasses or sunglasses unless it is for religious or medical purposes. You will need to provide us with a letter explaining the reason for wearing headwear and/or glasses.
  4. Authorization Letter. The Authorization Letter must be signed and dated. This letter allows us to expedite your new passport application. Please print two copies.
  5. Proof of Identification. Print copy of your drivers’ license or any other federal or state- issued photo ID. Photo ID needs to be valid and issued six months or older. Print front and back.
  6. Proof of US Citizenship. Provide an original state-issued or certified copy of your birth certificate or certificate of naturalization. All original / certified documents will be return with passport. No short forms or abstracts of Birth Certificate will be accepted.
  7. Government Fee. A check or money order, payable to the “U.S. Department of State”, of $170 must be provided along with the documents you are submitting. Temporary checks are not acceptable. A separate execution fee of $25 is due upon signing your DS-11 form.
  8. Acceptance Agent/Sealed Envelope. You must submit all materials to an approved acceptance facility to be executed by a federal official known as an Acceptance Agent.
